Hello,
In the attached file you will find a blank template workable with OpenOffice Draw. Open Office is a free suite that you can download at
http://www.openoffice.org/
You can use the Draw program to set up the layout at your will. The file you have to use is the .odg file. I have joined a jpg to show the current layout.
You can put your background on the BKGND layer. Put it visible and printable as in the current file I have disabled it.
